BENEFITS

VACATION : All employees receive vacation upon completion of 500 hours of work at the following rate:

• Employees with a hire date prior to July 1, 1992 shall accrue approximately two (2) weeks of paid vacation per year accrued at the rate of one (1) hour of vacation time for every twenty-five(25) hours worked.
• Employees with a hire date of July 1, 1992 or later, shall accrue approximately one (1) week of paid vacation per year accrued at the rate of one (1) hour of vacation time for every fifty (50) hours worked, and:

SICK LEAVE : All employees receive sick leave upon completion of 500 hours of work at the following rate:

• Employees with a hire date prior to July 1, 1992 shall accrue sick leave at the rate of one (1) Hour of sick leave for every twenty-one (21) hours worked.
• Employees with a hire date of July 1, 1992 or later, shall accrue approximately one (1) week of paid sick leave per year at the rate of one (1) hour of paid sick leave for every fifty (50) hours worked.

MILEAGE : reimbursement shall be at the rate of $.35 per mile.

HEALTH INSURANCE : Health insurance coverage shall be for full-time employees, those working an average of twenty-five (25) hours per week or more or fifty-four (54) hours a pay period wherein the Company will pay 80% of the monthly premium and the employee paying 20%. Part-time employees may purchase the coverage at 100% of the monthly cost.

DENTAL INSURANCE: Full-time employees shall be entitled to the Company paid dental insurance wherein 80% of the monthly premium is paid by the employer. Part-time employees may purchase the dental insurance at full cost.

HOLIDAYS : Full-time employees (those averaging twenty-five (25) hours a week or more or fifty-four (54) hours a pay period, shall receive the following paid holidays: (Must work 108 hours the previous two pay periods.)

2. Presidents' Day
3. Memorial Day
4. Independence Day
5. Labor Day
6. Thanksgiving Day
7. Christmas Day

PREMIUM PAY FOR CONTAGIOUS DISEASES : There shall be premium pay in the amount of $1.50 per hour, over and above the regular hourly rate of pay, for every hour worked, for service to clients with contagious diseases.
